Senior RN Case Manager: Lead Home-Based Elder Care

PACE Southeast Michigan is seeking a Registered Nurse Case Manager (RNCM) to lead in-home care coordination for frail elders with complex needs. You will assess, plan, implement, and evaluate nursing care with a focus on autonomy and family involvement. Responsibilities include collaborating with a multidisciplinary team, educating participants and caregivers, and supervising nursing staff. A Michigan RN license and driving are required; BLS and infection control training are expected. #J-18808-Ljbffr PACE
