Registered Nurse (RN), Personal Home Care Services Suffolk County, New York | Full-Time | Monday – Friday Care Where It Matters Most—At Home At Elara Caring, we believe the best care happens at home. As a Personal Care Services Registered Nurse RN, you’ll work independently while making a meaningful impact—supporting patients, guiding caregivers, and ensuring care is delivered safely and effectively in the home. What You’ll Do Conduct in-home patient assessments and develop care plans Supervise and support caregivers to ensure quality care delivery Perform start-of-care and supervisory visits Monitor patient condition and adjust care plans as needed Educate patients, families, and caregivers Ensure compliance with state, federal, and company standards Maintain accurate and timely clinical documentation Partner with clinical teams and case managers to coordinate care What We’re Looking For Active RN license (required) in the state of New York Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing Minimum 1 year of nursing experience (home care preferred) Strong clinical assessment and decision‑making skills Excellent communication and organizational abilities Ability to work independently in a fast‑paced environment Reliable personal transportation for field visits Bilingual skills a plus Autonomy in the field with strong clinical support 401(k) with employer match + generous PTO Tuition reimbursement + continuing education Career growth opportunities in a national organization This isn’t just a nursing job—it’s a leadership role in the field, where you directly impact patient outcomes, support caregivers, and help shape care delivery in your community.
