LVN - Licensed Vocational Nurse - Shifts: Mon-Fri 11pm-7am Full-time Location: Santa Clarita, CA Hourly rate: $27-$30 Prime Home Health is a leading provider of skilled in‑home nursing care. We are looking for passionate, empathetic, motivated, and committed Licensed Vocational Nurses (LVN) to provide quality care to home‑based patients. Responsibilities Provides direct patient care as defined in State's Nurse Practice Act. Implements plan of care initiated by the registered nurse. Provides accurate and timely documentation consistent with the plan of care. Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care. Prepares and coordinates clinical notes and progress notes. Works one‑on‑one with patients and their families, meeting positive patient care outcomes. Qualifications Current CA Vocational Nurse License. New graduates accepted under the Flex Waiver Program. Current TB test or chest x‑ray (if applicable). Current CPR/BLS certification.
