Job Description
Pediatric Case Manager RN

Assignment Duration

This is a 13-week contract position.

Schedule

5x8 Days, with shifts from 07:00 - 17:00. Weekend shifts required every other weekend.

Job Description

The Pediatric Case Manager RN will coordinate care for pediatric patients, focusing on discharge planning and case management. Candidates must have experience in pediatric care and be familiar with Epic charting systems. The role requires strong communication skills and the ability to work collaboratively with interdisciplinary teams.

Responsibilities

• Manage care coordination for pediatric patients.
• Conduct discharge planning and implement care plans.
• Collaborate with interdisciplinary teams including social services, pharmacy, and physical therapy.
• Utilize Epic for patient documentation and coordination.
• Ensure compliance with CMS guidelines and medical necessity determinations.

Required Experience / Certifications / Licensure

• Active AK RN license is required.
• Minimum of 2 years of pediatric nursing experience is required.
• ACM certification is preferred.
• Experience with Epic charting is mandatory.
• Acute care experience is necessary.
