Allied Technician - Physical Therapist

Physical Therapist (PT)

We are seeking a skilled Physical Therapist (PT) to join our Long Term Care (LTC) team. The successful candidate will be responsible for evaluating, rehabilitating, and developing appropriate care plans for patients with physical functioning disorders and wound care, ensuring compliance with all regulatory and clinical standards.

Key Responsibilities

• Screen, evaluate, and rehabilitate patients with physical functioning disorders and/or wound care.
• Develop and implement an appropriate plan of care based on clinical assessments and evidence-based practice.
• Administer various rehabilitation procedures, including manual techniques, ambulation, therapeutic exercises, and modalities.
• Utilize supportive and assistive devices and provide wound care as required.
• Assess patient progress and adjust treatments accordingly in alignment with regulatory requirements.
• Evaluate treatment outcomes to assess effectiveness and modify services as necessary.
• Provide consultation and counseling to patients, families, caregivers, and other service providers.
• Communicate effectively with supervisors and other health team members regarding patient progress and treatment plans.
• Prepare comprehensive discharge summaries of services provided in line with regulations.
• Document all required information in the electronic documentation system and maintain patient records as per company policy.
• Accurately record treatment minutes for services rendered.
• Participate in required meetings, including patient care conferences and rehabilitation meetings.
• Engage in facility and company-wide trainings, in-services, and conferences.
• Acquire necessary durable medical equipment (DME) or supplies in compliance with regulatory standards.
• Supervise and support physical therapist assistants, rehabilitation aides, and/or students.
• Adhere to the standards and ethics outlined by the APTA and state-required practice acts.

Qualifications

• A minimum of 1 year of Physical Therapy experience is required.
• Experience in Long Term Care (LTC) or Rehabilitation is mandatory.
• Must possess a valid Massachusetts state license to practice as a Physical Therapist.
