As a Licensed Practical Nurse (LPN) at Aperion Care Wesley, you will play a vital role in delivering exceptional care to our residents. Our state-of-the-art facility in Chicago, Illinois, offers a supportive environment where you can grow professionally and make a meaningful difference in the lives of our patients.
Responsibilities:
• Provide direct patient care, including administering medications, wound care, and vital sign monitoring
• Develop and implement individualized care plans to meet the unique needs of each resident
• Collaborate with the interdisciplinary care team to ensure seamless transitions and optimal outcomes
• Document patient progress and communicate effectively with families and healthcare providers
• Participate in quality improvement initiatives to continuously improve our care services
