Key Responsibilities

Deliver physical therapy treatments to patients in a Skilled Nursing Facility (SNF) setting under the supervision of a licensed Physical Therapist
Assist in implementing individualized treatment plans to support patient recovery and functional goals
Monitor and document patient progress, reporting changes in condition to the supervising Physical Therapist
Educate patients and caregivers on therapeutic exercises and safe mobility techniques

Required Qualifications

Active Physical Therapy Assistant (PTA) license in the state of Illinois
Graduate of an accredited Physical Therapist Assistant program
Current CPR/BLS certification
Experience working in a clinical or rehabilitative care setting

Preferred Experience / Skills

Prior experience working in a Skilled Nursing Facility (SNF) environment
Familiarity with geriatric patient populations and long-term care rehabilitation protocols
