Your Role as a Physical Therapist

As a Physical Therapist, you will provide skilled, individualized rehabilitation services designed to help residents maximize their physical function, improve mobility, and achieve meaningful therapeutic goals.

Key Responsibilities

• Evaluate referred residents and determine their rehabilitation needs.
• Assess medical condition, functional capabilities, limitations, restrictions, and rehabilitation potential.
• Develop and implement individualized treatment plans based on each resident's needs, capacity, and tolerance.
• Establish realistic, measurable, and achievable therapeutic goals.
• Provide skilled physical therapy services in accordance with physician orders and established plans of care.
• Monitor resident progress and modify treatment approaches as clinically appropriate.
• Educate residents and families on appropriate physical therapy techniques, exercises, safety strategies, and functional activities.
• Complete accurate and timely evaluations, weekly progress notes, re-certifications, treatment documentation, and discharge summaries .
• Collaborate with nursing, physicians, rehabilitation staff, and other disciplines to provide coordinated, patient-centered care.
• Maintain confidentiality and comply with HIPAA, Federal Residents' Rights, and applicable regulatory requirements .
• Perform other duties as assigned by the Director of Rehabilitation.

Qualifications & Requirements

• Graduate of an accredited Physical Therapy program .
• Current and valid Physical Therapist license/registration in the state of practice.
• Strong clinical, communication, documentation, and organizational skills.
• Compassionate, patient-focused approach to resident care.
• Ability to work effectively as part of an interdisciplinary team.
• Long-term care and/or geriatric rehabilitation experience preferred , but not required.
• Experience with Case Mix Index (CMI) and PPS scheduling preferred , but not required.
