Family Case Manager Schedule - Tuesday through Saturday

General Responsibilities: Under the direction of the DV Shelter Manager, the Family Case Manager provides trauma-informed case management and supportive services to families residing in the emergency shelter and transitional housing programs. The Family Case Manager works directly with children who have experienced trauma and their parents to promote safety, stability, and healthy family functioning.

 

General Requirements: Bachelor's degree or equivalent combination of education and relevant experience demonstrating comparable knowledge and skills, one year in a non-profit setting; strong organizational skills with exceptional attention to detail, ability to work both independently and collaboratively as part of a multidisciplinary team, demonstrated knowledge of domestic violence, trauma, and the impact of violence on women, children, and families, demonstrated commitment to providing trauma-informed, client-centered, culturally responsive, and strengths-based services. Completion of Agency Training within 6 months of employment,

 

AREAS OF RESPONSIBILITY

 

I. CHILDREN'S SERVICES

1. Provide trauma-informed, developmentally appropriate services for children residing in the emergency shelter and transitional housing programs, including planning and facilitating educational, recreational, life skills, and therapeutic activities that promote healing, resilience, and healthy development.
2. Support and strengthen the parent-child relationship by providing parents with education, coaching, and resources related to positive parenting practices, child development, behavior management, establishing routines, and the effects of trauma on children.
3. Facilitate individual and group activities for children that promote emotional expression, social skills, conflict resolution, safety planning, and healthy coping strategies in accordance with program goals.
4. Ensure compliance with all agency policies, procedures, licensing standards, confidentiality requirements, and legal and ethical mandates related to client safety, welfare, and mandatory reporting obligations.

II. CASE MANAGEMENT

1. Develop and implement individualized service plans for each child in collaboration with the parent or guardian to address developmental, educational, behavioral, and emotional needs, and coordinate referrals to appropriate community resources as needed.
2. Maintains client case files and related documents, Accurate and timely data entry of all services in agency’s database.
3. Collaborate with shelter staff, case managers, schools, mental health providers, and other community partners to advocate for children's educational, behavioral, medical, and social service needs.
4. Liaison with community agencies that serve children, such as Alameda County First 5 etc. Monitor the safety and well-being of children in the shelter environment, promptly address concerns, and communicate significant observations to the DV Shelter Manager and appropriate team members.

III. SHELTER OPERATIONS

 

As directed by the DV Shelter Manager and based on operational needs, provide direct services to shelter residents and community clients, including but not limited to:

1. Respond to the 24-hour crisis hotline by providing crisis intervention, safety planning, emotional support, information, referrals, and resource coordination.
2. Conduct client intakes, assessments, and program orientations in accordance with agency policies and procedures.
3. Participate in shelter operations, including responding to client emergencies, supporting shelter activities, and assisting with other duties necessary to maintain the safe and effective operation of the shelter.
4. Work assigned/rotating shifts to support the shelter’s operational requirements.

 

IV. GENERAL TASKS

 

1. Attend meetings including weekly staff meetings and Board/Staff meetings.
2. Participate in community fundraising and special events as required.
3. Will serve as back-up staff once each quarter for the crisis line and shelters on a rotating basis.
4. Must have an insured automobile that the employee will use as necessary for agency-related business

V. OTHER TASKS AS ASSIGNED BY THE DIRECTOR OF DOMESTIC VIOLENCE SERVICES OR THE EXECUTIVE DIRECTOR.
