Description: The RN implements professional nursing practice centered on the patient as a whole person. The focus is on the creation of a caring environment centered on healing and wellness. Nursing practices are based on evidence, current research, hospital policies, regulatory standards, and professional guidelines. Clinical decision-making incorporates all four phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through helping relationships among nursing staff and across other disciplines. The RN coordinates, ensures, and provides direct patient care activities on the unit. This includes the provision of nursing rounds, assessments, and treatments. The RN facilitates admission, transfer, and discharge activities. The RN provides patient and family-centered education, care coordination, and individualized treatment planning as applicable. The RN performs and documents comprehensive assessments in accordance with the RN's scope of practice and hospital policies and procedures. The RN considers the patient’s culture, age, and developmental level. Patient risk factors and presenting complaints are identified. The RN obtains patient health information from family or others as necessary. Objective and subjective data is collected. The RN verifies initial admission assessments if obtained by other members of the healthcare team. A problem list is developed, prioritizing patient needs based on input from the inter-professional team as appropriate. Actions and interventions relevant to the patient’s culture and age are designed. Desired outcomes are determined in coordination with the patient and family. The RN fully implements patient care therapies and procedures within the scope of practice. Procedures, treatments, and processes are explained to both the patient and family. The RN initiates actions to alleviate patient stress and discomfort. Changes in patient condition are recognized, responded to, and communicated appropriately. Ongoing assessments and responses to procedures and therapies are conducted and documented. The RN delegates tasks appropriately and oversees team members in the performance of activities regarding patient care. The accuracy and completeness of delegated patient care provided by other team members are monitored. Educational needs of patients related to prioritized problems are identified and addressed. The RN facilitates the patient discharge process by executing discharge orders in a timely manner. Potential discharges are identified, and discharge teaching is performed. Input into patient/nurse assignments is provided based on available staffing, unit activity, and patient acuity. Active participation in unit-based activities in support of organizational values and priorities is demonstrated. The RN promotes a positive group dynamic and healing environment. Nursing excellence is assured in accordance with accepted standards, evidence-based practice, and a caring philosophy. Participation in department and unit quality improvement activities and core measures is encouraged. A holistic perspective regarding patient care is utilized.
