Speech Pathologist | Nursing Home

Role Overview: The Speech-Language Pathologist evaluates, develops, and implements individualized treatment plans for patients with speech, language, cognition, voice, and dysphagia deficits in a home setting. They provide direct therapy, select and interpret diagnostic assessments, and modify intervention strategies based on ongoing evaluations. Key Responsibilities Assess patients' functional levels through diagnostic tests. Develop and adjust personalized treatment programs aligned with patient needs and goals. Deliver therapy services across various functional settings. Coordinate care with interdisciplinary teams, including families and physicians. Document evaluation results, treatment plans, and progress notes accurately. Participate in program development and supervise clinical interns. Qualifications & Skills CCC-SLP certification from ASHA or active clinical fellowship with certification within one year. State licensure as a Speech-Language Pathologist. Preferred 1-3 years of experience in clinical settings such as hospitals, SNFs, outpatient, or home health. Effective communication, visual acuity, and ability to work collaboratively.
