We are seeking an experienced, compassionate, and mission-driven Care Coordination Supervisor to play a key leadership role in an innovative integrated behavioral health and healthcare initiative. This position offers the opportunity to improve health outcomes for individuals with behavioral health, substance use, and co-occurring conditions through coordinated, person-centered care.
The Care Coordination Supervisor partners closely with program leadership to support interdisciplinary treatment teams, provide care coordination services, supervise project staff, and ensure the delivery of high-quality, culturally responsive care.

Position Summary

The Care Coordination Supervisor collaborates with program leadership to implement and oversee an integrated behavioral health and healthcare initiative. The role provides direct care coordination services, supervises project staff, and supports multidisciplinary treatment teams in delivering strengths-based, recovery-oriented, and culturally responsive services.
This position serves as a key liaison between behavioral health providers, primary care practices, specialty care providers, and community resources to ensure clients receive coordinated and comprehensive care.
Essential Responsibilities
Program Leadership
  • Partner with program leadership to implement and advance integrated healthcare initiatives and program objectives.
  • Co-lead multidisciplinary care teams consisting of clinical leaders, psychiatry providers, case managers, medical assistants, nurse care managers, and other healthcare professionals.
  • Support program operations, workflows, and quality improvement activities.
Care Coordination
  • Provide integrated care coordination to an assigned panel of clients.
  • Facilitate access to primary care, specialty care, behavioral health services, and community-based resources.
  • Monitor and support clients with chronic health conditions to ensure appropriate treatment and follow-up.
  • Conduct and review healthcare screenings and assessments.
  • Ensure accurate collection, tracking, and documentation of health indicators including blood pressure, body mass index (BMI), laboratory results, and other required measures within the electronic medical record (EMR).
Community Collaboration
  • Develop and maintain collaborative relationships with primary care providers, specialty care providers, hospitals, and community healthcare organizations.
  • Coordinate the exchange of client information, with appropriate consent, to support continuity of care.
  • Collaborate with external partners, evaluators, and stakeholders to support client outcomes and program reporting requirements.
Staff Supervision & Team Development
  • Provide supervision, guidance, and support to case managers, medical assistants, and other project staff.
  • Delegate assignments and monitor performance to ensure compliance with program standards and best practices.
  • Promote a collaborative, accountable, and client-centered team culture.
  • Support staff development and ongoing training initiatives.
Quality Improvement
  • Promote evidence-based practices in both behavioral health and primary care settings.
  • Utilize data and performance metrics to drive decision-making and continuous quality improvement efforts.
  • Assist in monitoring program outcomes and ensuring compliance with operational and regulatory requirements.
Additional Responsibilities
  • Perform other duties as assigned to support the success of the program and organization.
Qualifications
Required Qualifications
  • Bachelor's Degree in Nursing and active Registered Nurse (RN) license with 1-2 years of experience in behavioral health, integrated care, care coordination, or a related healthcare setting;
OR
  • Active Licensed Practical Nurse (LPN) license with 2-4 years of related experience, including staff supervision or leadership responsibilities.
  • Knowledge of integrated care and whole-person healthcare models.
  • Experience working with individuals experiencing behavioral health, substance use, and co-occurring disorders.
  • Strong clinical judgment, problem-solving abilities, and decision-making skills.
  • Ability to manage sensitive information with professionalism, discretion, and confidentiality.
  • Experience utilizing evidence-based practices.
  • Proficiency with Microsoft Office and Electronic Medical Record (EMR) systems.
  • Excellent written, verbal, and interpersonal communication skills.
  • Strong organizational skills with the ability to manage multiple priorities effectively.
  • Commitment to cultural humility, equity, inclusion, and person-centered care.
  • Ability to maintain professionalism in a fast-paced and evolving environment.
Preferred Qualifications
  • Bilingual/Bicultural (Spanish-English).
  • Experience in community behavioral health, integrated care, primary care, or population health settings.
  • Experience working with multidisciplinary treatment teams.

The pay range for this position is $60000.00 - $90000.00/yr.
