Lead Care Manager

Aledade is seeking a Care Manager to support primary care practices under the Primary Care Program. You will conduct comprehensive assessments, coordinate care, and leverage the Aledade App to manage high-risk patients and improve chronic-care outcomes. You will work with Maryland/DC practices, provide education and coaching over the phone, and collaborate with interdisciplinary teams to drive value-based care initiatives. #J-18808-Ljbffr
