Beyond Faith Homecare & Rehab is currently accepting applications from compassionate and dedicated therapy professionals, including Physical Therapists (PTs), Occupational Therapists (OTs), and Speech‑Language Pathologists (SLPs). This posting serves as a general application for current and future therapy opportunities throughout our service area. We are seeking licensed therapy professionals who are passionate about delivering high‑quality, patient‑centered care in the home. Responsibilities Assist clients with tasks within the scope of job description based on the Plan of Care (POC). Communicate effectively with administrative staff regarding client/family needs assessments, including reporting any significant changes in client condition. Provide care as outlined in the Care Plan and accurately document the care provided. Contribute to continuity of care, effective communication between all care team members, and regular participation in case conferences/Team Meetings. Apply concepts of infection control and universal precautions in coordination/performing client care activities to protect both staff and patients. Demonstrate commitment and professional growth by participating in in‑service programs and maintaining/improving competency. Complete training or in‑services in a timely manner as assigned by your supervisor. Meet mandatory continuing education requirements of the Agency/licensing board. Qualifications Experience: One (1) year as a therapist in a home health setting. Successful completion of formal training in nursing arts may be substituted for the qualifying experience, subject to applicable State law, regulation, or rules. Education: High School Graduate or equivalent. Licensing & Certifications: Current CPR certification. Current driver’s license in good standing; responsible for renewing before expiration. Reliable transportation and auto liability insurance. Physical Requirements: Prolonged standing/walking required; ability to lift/transfer up to 50 lbs.; work under stressful conditions to meet deadlines; meet patient/family psychosocial needs; requires hand-eye coordination and manual dexterity. Environmental/Working Conditions: Works in patients’ homes in various conditions; possible exposure to blood‑borne pathogens, bodily fluids, and infectious diseases; occasional exposure to unpleasant weather; PRN emergency call; ability to travel locally and work a flexible schedule.
