Job Description
LTC/SNF LPN

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) for our Long-Term Care/Skilled Nursing Facility. This role involves providing exceptional patient care to elderly individuals with multiple diagnoses.

Job Details

Unit Type: LTC/SNF
Shift Length: 12 hours
Guaranteed Hours: 36 hours per week
Years of Experience Required: 1 year
Certifications: BLS Certification required
Shifts: 6:00 AM - 6:30 PM; every other weekend and holiday

Facility Information

Bed Capacity: Current census is 42 beds
Patient Ratios: Varies; days shift is 1:21, at most 1:42
Electronic Medical Records: PCC system
Weekend Requirement: Every other weekend
Holiday Requirement: Every other holiday
Orientation: 1 day of location-specific orientation followed by up to 3 days on the floor with staff
Common Patient Diagnoses: Primarily elderly patients with multiple diagnoses

Responsibilities

The LPN will be responsible for providing high-quality nursing care to residents, monitoring their health status, administering medications, and collaborating with other healthcare professionals to ensure the best outcomes for patients.

Additional Notes

Local travel may be required within 80 miles.
No additional charge differentials or call back duties apply.
Resignation with less than 14 days' notice will incur a fee.
