Advanced Practice Provider (Nurse Practitioner or Physician Assistant)An established outpatient healthcare practice is seeking an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join a collaborative clinical team dedicated to delivering comprehensive patient care. This position combines preventive health, wellness, acute care, and chronic disease management, with a strong emphasis on patient education and long-term health improvement through lifestyle-focused treatment strategies. The provider will work closely with a supervising physician and interdisciplinary team to deliver evidence-based care, promote wellness initiatives, and help patients achieve better health outcomes.Key ResponsibilitiesPerform pre-employment physical examinations and annual wellness assessmentsEvaluate, diagnose, and treat a variety of acute illnesses and injuriesManage chronic medical conditions using evidence-based clinical guidelinesDevelop and implement individualized treatment plans in collaboration with the supervising physicianConduct preventive health screenings and support wellness initiativesCounsel patients on lifestyle modifications, disease prevention, and health maintenanceOrder, review, and interpret laboratory and diagnostic studiesMonitor treatment effectiveness and adjust care plans as appropriateCoordinate referrals to specialists and other healthcare providers when necessaryCollaborate with medical leadership and stakeholders regarding patient care mattersSupport occupational health services, including oversight of respirator fit testing and hearing evaluations performed by clinical staffMaintain accurate and timely electronic medical records documentationAssist with clinic operations and additional responsibilities as neededRequired QualificationsBoard Certified Nurse Practitioner (NP) or Certified Physician Assistant (PA)Active and unrestricted state licenseMinimum of one year of experience in one or more of the following settings:Family MedicineInternal MedicineUrgent CareCurrent BLS certificationStrong written and verbal communication skillsAbility to work efficiently in a fast-paced outpatient environmentCommitment to delivering patient-centered careComfortable caring for pediatric patients in a clinical setting beyond routine wellness visits, milestone assessments, and vaccine managementPreferred QualificationsDEA licenseExperience in occupational health, employee health, or wellness-focused clinical settingsExperience working across multiple patient care environmentsExperience collaborating with multidisciplinary teams and organizational stakeholdersLocal ties to the Cumming, GA area or surrounding communitiesStrong interest in lifestyle medicine and utilizing lifestyle-based interventions as part of chronic disease management and preventive careIdeal Candidate ProfileBe local to the area or have strong familiarity with the communityBring experience from multiple healthcare settingsBe comfortable treating both adult and pediatric patientsHave experience managing pediatric medical concerns beyond routine wellness careEmbrace a lifestyle medicine approach, emphasizing nutrition, exercise, behavior modification, and preventive strategies alongside traditional treatment plansEnjoy building long-term patient relationships and promoting overall wellnessKey Skills RequiredPatient assessment and clinical decision-makingPreventive care and wellness managementAcute and chronic disease managementPediatric patient careLifestyle medicine and patient coachingEMR documentationInterdisciplinary collaboration
