Job Description
Embassy Manor in Edison, NJ is hiring a full-time Nurse Practitioner (NP) to join our clinical team. For over 20 years, Embassy Manor has been a trusted name in post-acute rehabilitation, long-term care, and memory care in the Edison community — and our Nurse Practitioners are at the center of how we deliver care. Prior nursing home experience is required for this Nurse Practitioner position.
This is a great fit for a Nurse Practitioner (NP) who wants to practice in a clinically sophisticated skilled nursing setting. Our facility runs physician-led specialty programs in cardiac care and CHF, stroke and neurological recovery, wound care, and pulmonary disease management — meaning our NPs manage genuinely interesting, medically complex patients alongside a devoted interdisciplinary team.
What You'll Do
Provide direct patient care, assessments, and treatment for residents and short-term rehab patients
Collaborate with attending physicians, nursing staff, and therapy teams on care planning
Manage medically complex patients within our specialty clinical programs
Support hospital readmission prevention and smooth care transitions
Document thoroughly and communicate with families as part of a resident-centered approach
Qualifications
Active NP (Nurse Practitioner) / APN (Advanced Practice Nurse) license in New Jersey
Minimum 6 months of experience as a Nurse Practitioner (NP)
Must have prior experience in a nursing home / SNF (Skilled Nursing Facility) / LTC (Long-Term Care) setting — this is required
