Hospice Case Manager

The ideal candidate will have a strong background in hospice, excellent communication skills, and a passion for patient advocacy.

• Conduct clinical assessments to determine hospice eligibility and patient needs.
• Serve as a liaison between referral sources, patients, families, and the hospice team.
• Educate patients and families about hospice philosophy, services, and care plans.
• Coordinate and schedule home visits, record reviews, and admission processes.
• Ensure timely, compliant admissions in accordance with Medicare and hospice guidelines.
• Collaborate closely with the interdisciplinary team to develop initial plans of care.
• Maintain accurate and up-to-date documentation in EMR systems.
• Provide crisis intervention and emotional support during the transition period.
• Represent Varco Hospice with professionalism and compassion to referral partners and healthcare facilities.
• Attend team meetings and participate in ongoing quality improvement efforts.

• Active RN license in the state of practice (required).
• Minimum 1 year of experience in hospice (required).
• Deep understanding of hospice regulations, eligibility, and best practices.
• Strong clinical judgment, assessment, and critical thinking skills.
• Compassionate and empathetic communication with patients and families.
• Highly organized and able to manage multiple priorities independently.
• Proficient in EMR systems and documentation protocols.
• Valid driver's license and reliable transportation.
• CPR certification preferred.
• Bilingual abilities are a plus.
