Hospice Case Manager

The ideal candidate will have a strong background in hospice, excellent communication skills, and a passion for patient advocacy.
Key Responsibilities- Conduct clinical assessments to determine hospice eligibility and patient needs.
- Serve as a liaison between referral sources, patients, families, and the hospice team.
- Educate patients and families about hospice philosophy, services, and care plans.
- Coordinate and schedule home visits, record reviews, and admission processes.
- Ensure timely, compliant admissions in accordance with Medicare and hospice guidelines.
- Collaborate closely with the interdisciplinary team to develop initial plans of care.
- Maintain accurate and up-to-date documentation in EMR systems.
- Provide crisis intervention and emotional support during the transition period.
- Represent Varco Hospice with professionalism and compassion to referral partners and healthcare facilities.
- Attend team meetings and participate in ongoing quality improvement efforts.
- Active RN license in the state of practice (required).
- Minimum 1 year of experience in hospice (required).
- Deep understanding of hospice regulations, eligibility, and best practices.
- Strong clinical judgment, assessment, and critical thinking skills.
- Compassionate and empathetic communication with patients and families.
- Highly organized and able to manage multiple priorities independently.
- Proficient in EMR systems and documentation protocols.
- Valid driver's license and reliable transportation.
- CPR certification preferred.
- Bilingual abilities are a plus.
