Hospice Social Worker

As a Hospice Social Worker, you will play a vital role in our interdisciplinary team, working closely with patients, families, and healthcare professionals to provide medical social services tailored to the patient's plan of care.

Job Summary

We are seeking a skilled and compassionate Hospice Social Worker to join our team in Marietta, Ohio. As a Hospice Social Worker, you will provide medical social services to patients and their families, conduct psychosocial assessments, develop care plans, and evaluate patient and family responses to interventions. You will also participate in IDG meetings, QAPI activities, and maintain accurate clinical records.

Responsibilities

• Conduct psychosocial assessments and develop care plans in collaboration with the healthcare team

• Provide medical social services to patients and their families, addressing emotional, social, financial, and environmental factors

• Coordinate with other healthcare professionals to ensure comprehensive care

• Maintain accurate and timely clinical records

• Evaluate patient and family responses to interventions and adjust care plans accordingly

• Participate in IDG meetings, QAPI activities, and other team meetings as necessary

• Collaborate with the healthcare team to develop and implement discharge plans

Requirements

• Must be a licensed driver with an insured automobile in good working order

• Ability to access patients' homes and perform physical activities such as walking, sitting, stooping, and standing

• Utilization of computer, telephone, and other office equipment

• Bachelor's degree in social work or related field, with at least 1 year of social work experience in a healthcare setting (or Master's degree with 1 year of experience preferred)

• Hospice experience preferred

• Ability to communicate effectively, both verbally and in writing
