Physical Therapist Early Intervention Evaluator

Overview Thrive By 5 is a pediatric therapy agency providing services to children birth-3 years old. We are a growing agency seeking strong Physical Therapists to provide early intervention evaluations in New York City (Manhattan, Brooklyn, Queens, Bronx, and/or Staten Island). We take great pride in the work we do and have a strong reputation in the field. Responsibilities Utilize DOH approved, age appropriate, testing tools to evaluate infants/toddlers who have been referred to the early intervention program Provide a written report and other documentation required by the early intervention program Collaborate with other evaluators to support the multi-disciplinary evaluation process Other duties as assigned. Qualifications Masters or Doctorate degree in Physical Therapy NYS License in Physical Therapy required Must demonstrate excellent written, communication, and interpersonal skills Must be able to collaborate with the clinical team and all related service providers Our mission is to provide the best possible services to children in their natural environments.
