RN MDS Coordinator

Full Time

On-Call rotation is required.

Required Qualifications:

An unencumbered RN (Registered Nurse) nursing license in the State of Ohio.

Must have, as a minimum, one (1) year of experience with demonstrated success in a nursing position in the facility or in another related health care facility.

Experience with the MDS process.

Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations and guidelines pertaining to long-term care.

Knowledgeable of skilled nursing home regulations, procedures, laws, regulations and guidelines pertaining to long-term care.

Duties and Responsibilities including but not limited to:

MDS scheduling and completion per guidelines including coordinating care plan development and completion with interdisciplinary team.

Ensures care plans are reviewed quarterly and updated as needed to reflect current resident status with individualized problems, goals and interventions.

Oversees and monitors MDS documentation and charting requirements that support services provided to meet billing requirements including state specific requirements for supportive documentation.

Completes chart audits to assess the documentation to support skilled Medicare, Managed Care and Medicaid.

Ability to be accurate and detail oriented.

Demonstrate clinical assessment skills to meet all job requirements.

Performs other duties implementing the role of an MDS Coordinator.
