Physical Therapist (PT) | Full-Time & Part-TimeAll Heart Home Care | Columbia, TNAll Heart Home Care is seeking compassionate and skilled Physical Therapists (Full-Time and Part-Time) to join our growing home health team in the Columbia, Tennessee area. This role provides the opportunity to deliver personalized, one-on-one care to patients in the comfort of their homes while enjoying a flexible schedule and a supportive work environment.If you are passionate about helping patients improve their mobility, independence, and quality of life, we encourage you to apply.Key ResponsibilitiesPerform in-home evaluations to assess patient needs and develop individualized treatment plansProvide skilled physical therapy services to improve mobility, strength, balance, and overall functionMonitor patient progress and modify treatment plans as appropriateEducate patients and caregivers on home exercise programs, safety measures, and injury preventionComplete accurate and timely clinical documentation in accordance with company policies and state regulationsCollaborate with physicians, nurses, case managers, and other healthcare professionals to ensure coordinated patient careTravel to patient homes within the assigned service areaQualificationsDoctor of Physical Therapy (DPT) or equivalent from an accredited programActive Tennessee Physical Therapist licenseCurrent CPR certificationValid driver's license, reliable transportation, and current automobile insuranceStrong clinical assessment and communication skillsAbility to work independently and manage time effectivelyPrevious home health experience preferred, but not requiredAvailable PositionsFull-Time Physical TherapistPart-Time Physical TherapistAbout All Heart Home CareAt All Heart Home Care, we are committed to providing high-quality, compassionate care that allows patients to thrive in the comfort of their own homes.
