Travel Physical Therapist (PT) – Long Term Care (LTC)

Location: Pampa, TX
Schedule: 8-Hour Day Shifts
Guaranteed Hours: 40 Hours per Week
Weekend Requirement: None
On-Call Requirement: None

Responsibilities

• Perform comprehensive patient evaluations, assessments, and functional mobility screenings.
• Develop and implement individualized treatment plans based on patient needs and physician orders.
• Provide therapeutic exercises, gait training, balance training, mobility interventions, and pain management techniques.
• Monitor patient progress and modify treatment plans as needed to achieve optimal outcomes.
• Educate patients, families, and caregivers on treatment goals, home exercise programs, and injury prevention strategies.
• Coordinate discharge planning and continuity of care throughout the rehabilitation process.
• Supervise and delegate appropriate tasks to therapy assistants and support staff in accordance with state regulations.
• Maintain accurate, timely, and compliant documentation.
• Collaborate with interdisciplinary healthcare teams to ensure patient-centered care.

Requirements

• Minimum 2 years of Physical Therapist experience in a Long-Term Care setting required.
• Active Texas Physical Therapist License or Compact License required.
• Strong assessment, treatment planning, and documentation skills.
• Experience working with geriatric and rehabilitation populations preferred.
