Key Responsibilities of the RN - Case Management

• Perform comprehensive assessment of high risk members
• Collaborate with primary care providers for individualized care plans
• Reassess care levels and put support in place for treatment plan compliance
• Coordinate care transitions with healthcare and claims professionals
• Engage specialty resources for optimal outcomes
• Maintain detailed records of clinical, functional, and fiscal outcomes

Requirements of the RN - Case Management

• RN KY license
• Recent Home Health experience, skilled in OASIS and HCHB
• Associates or Bachelor's Degree in Nursing or related field
• Current, unrestricted state Registered Nurse license
• Ability to analyze complex medical information and make decisions
