Adaptive Home Health
- Role: On Call Registered Nurse (RN) - Case Manager
- Comp: $64.10 per point
- Additional compensation for any services provided after hours.
- $0.67 cents per mile for mileage reimbursement
- Schedule: On Call After Hours/ Part Time (as needed)
- Location: Field-based
- Carry a high-quality caseload: at target visit quality, with OASIS accuracy that holds up to review .
- Own your patients' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality.
- Be a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent.
- Set the standard others copy: your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows.
- Comprehensive in-home assessments and skilled documentation (OASIS, SOC/recert/ROC, discharge).
- Direct, high-acuity patient care, with a focus on wound care
- Building and managing the plan of care with physicians; supervising LPNs and HHAs on your cases.
- Being the face of Adaptive to patients, families, and the referral sources who feed your pod.
- Dedicated clinician: home health experience strongly preferred
- High ownership: you treat your caseload like your own panel and your pod like your own business.

- Tech-forward: You want to leverage software to accelerate the charting process, and you'll give us blunt feedback to make it better

- Reliable transportation, valid driver's license, auto insurance
