Speech-Language Pathologist - Skilled Nursing Facility

Overview We are seeking a skilled and compassionate Speech-Language Pathologist to join a Skilled Nursing Facility in Modesto, California. This role focuses on evaluating and treating patients with speech, language, communication, cognitive, and swallowing disorders within a geriatric and post-acute population. You will work closely with interdisciplinary healthcare teams to help patients improve communication abilities, cognitive function, and safe swallowing skills while enhancing overall quality of life. Responsibilities Evaluate, diagnose, and treat speech, language, communication, cognitive, and swallowing disorders in a Skilled Nursing Facility setting Develop and implement individualized treatment plans based on patient needs and clinical findings Provide therapy focused on speech production, cognitive communication, memory, language, and dysphagia management Collaborate with physicians, nurses, therapists, dietitians, and interdisciplinary teams to support patient recovery Educate patients and caregivers on communication strategies, swallowing precautions, and home exercise programs Monitor patient progress and modify treatment plans as needed Maintain accurate and timely documentation in compliance with facility and regulatory standards Requirements Active state license as a Speech-Language Pathologist in California ASHA CCC-SLP certification preferred or required depending on facility policies Current BLS/CPR certification preferred Minimum of 1 year of recent SLP experience preferred, ideally in a SNF, rehabilitation, or geriatric setting Strong communication, clinical reasoning, and patient care skills Benefits Competitive compensation package Continuing Education (CE) reimbursement Supportive and collaborative clinical environment Opportunity to make a meaningful difference in patient recovery and quality of life #J-18808-Ljbffr
