Registered Nurse (RN) MDS Coordinator
Job Summary:

We are seeking a highly skilled and dedicated Registered Nurse (RN) MDS Coordinator to join our healthcare family. As an integral member of our team, you will be responsible for coordinating and managing the Minimum Data Set (MDS) process for our residents, ensuring seamless transitions and exceptional care outcomes.
Responsibilities:
Coordinate and manage the MDS process, including data collection, validation, and submission
Ensure accurate and timely completion of MDS assessments, care plans, and progress notes
Collaborate with interdisciplinary teams to develop and implement individualized care plans
Provide education and training to staff members on MDS guidelines, regulations, and best practices
Ensure compliance with federal and state regulations, as well as accreditation standards
Participate in quality improvement initiatives and contribute to the development of quality metrics
Communicate effectively with residents, families, and healthcare providers to ensure seamless care transitions
Requirements:
Current licensure as a Registered Nurse (RN) in the state of Pennsylvania
