Job Title: RN: Case Manager Location: Williamsburg, VA Setting: Home Health Pay Range: 2162.60 - 2270.73 Schedule: 8:00-17:00 Mon-Fri, some On-Call required as needed Contract Length: 13 weeks Candidate Type: Travel Requirements: Virginia License or Compact License; Hospice experience a must; HCHB experience is a plus; Associates or Bachelor's Degree in Nursing or related field; Current, unrestricted state Registered Nurse license; Ability to analyze complex medical information and make rational decisions; Case Management certification Responsibilities:

• Perform comprehensive assessment of high-risk members
• Collaborate with primary care providers to ensure the implementation of an individualized, comprehensive care plan with specific interventions
• Address, identify, and continuously reassess cost-efficient, appropriate levels of care
• Coordinate care transitions and engage specialty resources as needed to achieve optimal resolution
• Keep detailed records of clinical, functional, and fiscal outcomes during the management process
