Social Worker (Program Coordinator)

Summary The Social worker who serve as the Program Coordinator at the GS-12 grade are administratively responsible for a clinic program providing treatment to patients in a major specialty, such as Spinal Cord Injury (SCI) Medical Foster Home, Community Residential Care, Community Nursing Home Program, Adult Day Health Care, Respite, Suicide Prevention Coordinator, Caregiver Support Coordinator, Local Recovery Coordinator, and other specialized programs. - The Coordinator administers information and analytical systems to evaluate and enhance the quality of services provided to patients.
- He/ She develops and implements information systems to track service activities including visits, referrals, inpatient days patient demographics, equipment utilization and patient satisfaction.
- The Coordinator develops, implements and evaluates clinical guidelines and protocols to establish appropriate utilization of services.
- The Social Work (Program Coordinator) performs the administrative and personal management function relative to staff supervised.
- HS/ She establishes guidelines and performance expectations for staff, which are clearly communicated through the formal employee performance management system.
- The Social Worker establishes and maintains effective therapeutic relationships with Veterans and their families.
- The Social Worker acts as a subject matter expert and liaison with community agencies.
- He/ She coordinates community-based services, including information and referral to or from other VA programs, government programs, and community agency programs.
- The Social Worker has knowledge and experience in the use of medical and mental health diagnoses, disabilities, and treatment procedures.
- The Social Worker (PC) identifies high-risk patients and provides appropriate intervention case management services.

- Must be proficient in written and spoken English.
- Subject to background/security investigation.
- Must be able to independently operate Government Vehicle.
- Must provide clean driving record report from the DMV during the pre-employment process.

- Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).

- English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f).
- Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work.
- Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level.
- Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring.
- Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
- Ability to supervise multidisciplinary staff assigned to the program.
- Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
- Ability to provide training, orientation, and guidance within clinical practice.
