Providing proactive clinical intervention and coordination of services, the full-time hourly North Carolina Licensed Post Transition Coordinator will support individuals in maintaining their homes within the community as part of the Transitions to Community Living program, working remotely from North Carolina. Key responsibilities Manage post-transition planning and support for members to ensure successful community integration and ongoing tenancy Coordinate rehousing efforts for members at risk of losing their housing, including leading Treatment Team meetings and developing Rehouse Plans Document all interactions and interventions in the Electronic Health Record and ensure compliance with reporting requirements Required qualifications Master's degree in a Human Services Field Valid North Carolina clinical licensure (social work, counseling, or psychology) Two years of post-Bachelor's degree experience with the population served if the Bachelor's degree is in Human Services; four years if in another field Proficiency in Microsoft Office products and Vaya information systems Knowledge of the MH/SU/IDD service array and relevant state and federal regulations
