LPN – Long Term Care

Schedule: Days

Assignment Details: Shared housing available (furnished, includes Wi-Fi and cable). No pets allowed. No smoking.

Requirements

• Current, unrestricted LPN license in Colorado
• Minimum of 3 years of experience in Long Term Care
• BLS/BCLS certification required
• MDS experience preferred

Responsibilities

• Provide direct nursing care to residents in a Long Term Care setting
• Administer medications and treatments as prescribed
• Assist with the development and implementation of individual care plans
• Monitor and document patient progress and response to treatment
• Collaborate with interdisciplinary team members to optimize resident care
• Educate residents and families about health management and care plans
