Occupational Therapists (OT) - Home Care

Overview The Home Care Occupational Therapist evaluates and develops treatment plans for patients of all ages to resume or adjust daily living activities. Collaborates with the health care team and modifies the plan of care as appropriate. Serves as a role model in the delivery of professional services and as a clinical resource for staff and students. Hours may vary depending on census and program needs. Qualifications Certified Licensed Occupational Therapist (OT) – Illinois Department of Financial and Professional Regulation (IDFPR) Proof of Auto Insurance Basic Life Support (BLS) within 30 days – American Heart Association (AHA) Driver's License – Illinois Secretary of State (ILSOS) Education: Bachelor’s Degree in Occupational Therapy (Required) Work Experience: [not specified] Responsibilities Review plan of care and orders with the primary physician; read back, complete, and transmit within the time frame established by policy. Educate patients and family members in a manner that overcomes barriers, matches learning capabilities, and meets fundamental needs. Document comprehensive details about patient visits so following staff can determine exactly what and how care was rendered, with a clear plan for the next contact. Document within guidelines and timeframes set by regulatory requirements and/or department policy. Update goals promptly; ensure goals are closed as met or changed as the patient’s condition progresses. Collaboratively communicate, initiate case conferences as necessary, and document interactions with other disciplines, MDs, care coordinators, insurers, etc. Document care plans and patient/family assessments, indicating evidence of therapy knowledge, assessment skills, and critical thinking. Anticipate and order appropriate quantity of supplies with sufficient lead time to prevent shortages between visits. Educate and document discharge planning prior to discharge; communicate with physician, insurance, outpatient visits, and community resources; provide discharge documentation to the patient. Complete audits and other assigned tasks within the assigned time frame. Evaluate the patient’s level of function within regulatory requirements by applying diagnostic and prognostic procedures as directed by the physician or podiatrist. Guide the patient in the use of therapeutic, creative and self‑care activities to improve function. Observe, record, and report to the physician or podiatrist the patient’s reaction to treatments and any changes in condition. Instruct other health team personnel, including OTAs, Home Health Aides and family members, in appropriate phases of occupational therapy. Communicate OTA assignments with the scheduler, prepare written instructions, and supervise assistants in the patient’s home. Understand all home care rules and regulations and comprehend their influence on case mix index. Complete OTA supervisory visits at least every six visits; complete HCA supervisory visits every fourteen days if only therapy in home. Complete comprehensive OASIS assessments, assess home‑bound status, complete occupational therapy evaluation in the patient’s home, and develop a plan of care. Communicate with patients to schedule appointment times and manage the daily schedule. Assess home safety and recommend equipment to improve the patient’s home safety. Educate family/caregivers on recommended home exercise program. Compensation and Benefits The compensation range for this position is $34.66 per hour – $59.62 per hour. The actual compensation offered to a candidate will be dependent on experience, qualifications, location, training, licenses, shifts worked and compensation model.
