Home Health Start of Care Registered Nurse (RN) 7 On 7 Off Join our team at Specialized Home Care, a leading provider of home health care services, as a Start of Care Registered Nurse (RN) working a 7 on/7off schedule. As a member of our team, you will play a critical role in delivering exceptional patient care, establishing individualized plans of care, and promoting positive clinical outcomes. Essential Responsibilities Complete Start of Care (SOC) visits and comprehensive patient assessments in accordance with physician orders, agency policies, and regulatory requirements. Perform and accurately document OASIS assessments within required timeframes. Develop individualized Plans of Care based on patient needs, physician orders, and clinical findings. Assess patients' physical, emotional, psychosocial, cognitive, environmental, and safety needs. Establish appropriate home health services and coordinate care with physicians, therapists, social workers, and other members of the interdisciplinary team. Schedule Full-Time 7 Days On / 7 Days Off Scheduled admission and Start of Care visits during assigned work week Weekend and holiday coverage required during scheduled rotation Qualifications Active Registered Nurse (RN) license in the state of practice. Minimum of two (2) years of nursing experience. Current CPR certification. Valid's driver's license, reliable transportation, and current automobile insurance. Ability to travel throughout the assigned service area. Strong clinical assessment, critical thinking, and organizational skills.
