Re-entry Services Coordinator

The Role: The Re-entry Services Coordinator works with individuals who are transitioning from incarceration to the community or are being supervised in the community by the Vermont Department of Corrections. The role of the Re-Entry Service Coordinator is to provide community based supports that are designed to promote the acquisition of life skills and access to resources which will support inclusion in the community and lead to permanent housing.

Position Available: 1

Location: In Person (Windham and Windsor Counties)

Full Time: Full-Time (35-40 Hours weekly)

Supervisor: Regional Re-Entry Services Team Lead

Responsibilities:

  • Engage in relationship building/rapport building with clients and provide services with a client centered, strengths-based approach.

  • Provide one to one support and counseling on life skills, tenancy skills, budgeting skills, housekeeping, skills.

  • Assist clients in navigating the housing path including housing search and applications, viewing available units, understanding lease agreements and tenant rights and responsibilities, assistance with moving in, etc.

  • Educate, inform, and advocate for clients regarding available benefits and entitlements (Social Security, Public Assistance, food stamps, Hireability/ICAN, etc.).

  • Maintain written and computerized records, compile reports and complete other program documentation (including case notes, face sheets, client files, application tracking, petty cash receipts, etc.).

  • Collect and maintain information related to participant’s housing history including: income and income history, rental history, criminal history and credit history (as these relate to obtaining/maintaining housing).

  • Support lease ups, move-in, move-outs and regular inspections pursuant to Housing quality standards between tenant, landlord and housing authority inspector.

  • Attendance at the annual All Staff and Programmatic Retreats is an expectation of employment with Pathways Vermont, unless excused by the employee’s manager/HR. Retreats are held on workdays, during normal working hours, and are fully paid.

  • Occasional travel to other counties when needed to support participants and community partners.

  • Other Duties as assigned

Qualifications:

  • Ability to work within a client centered, strengths-based framework. A compassionate understanding of the issues facing persons with mental health challenges or substance use issues, particularly as related to meeting basic needs of food, clothing, shelter and advocacy.

  • Excellent communication, writing, and computer skills

  • Ability to structure time, develop work priorities independently, and meet program expectations regarding documenting interactions.

  • Valid driver’s license, a reliable personal vehicle available for work purposes and current automobile insurance required.

  • Ability to lift and carry 50 pounds.

  • Willingness to self-reflect, learn, and practice new ways of being in relationships

  • Risk and discomfort tolerance

  • Continued employment is contingent upon successful PREA certification and satisfactory AHS Background check.

  • Lived experience of homelessness, mental health or other life challenges considered an asset.

Physical Requirements:

  • Occasional exposure to outside environmental conditions for community-based meetings.

  • Often meeting with persons in the community and/or visiting different businesses

  • Extending hand(s) and arm(s) in any direction at times when collecting paperwork, assisting with applications, etc.

  • Extensive reading responsibilities

  • Ability to ascend and descend stairs occasionally required (in instances when businesses and/or meeting places do not have an elevator)

  • Regular operation of a computer and other office machinery including frequent typing. Grasping. Applying pressure to an object with the fingers and palm such as stapling, completing hand-written forms, etc.

  • Frequent talking and listening in participation of exchanging ideas through spoken word and occasional receipt and giving detailed information through oral communication at normal speaking levels.

  • Occasionally lifting and carrying up to 30 pounds, occasionally exerting up to 20 pounds of force and/or up to 10 pounds of force frequently (opening doors at meeting places/businesses, etc.)

  • The worker is required to have visual acuity to operate motor vehicles

Compensation:

  • $22-$24 per hour
