About the Facility – Chalmers P. Wylie VA Ambulatory Care Clinic The Chalmers P. Wylie VA Ambulatory Care Center (VAACC) is a Joint Commission accredited, complexity level 2 facility serving Veterans in 13 counties of Ohio. The VAACC is the parent facility for a Veterans Readjustment Counseling Center (Vet Center) located in downtown Columbus. The VAACC has education, training, and clinical affiliations with multiple accredited colleges and universities. The facility has established medical school affiliations with Ohio University and The Ohio State University, with resident rotations in physical medicine, dermatology, ophthalmology, psychiatry, and internal medicine. Facility Address: 420 N James Rd Columbus, OH 43219ResponsibilitiesObtain patient history and pertinent family history.Assess patient and plan appropriate nursing care.Document plan of care and care given in patient's medical record.Verify transcription of physician's orders.Communicate pertinent patient data to charge nurse and/or physician as appropriate.Re-assess patient's condition and revise plans of care based on identified nursing problem.Possess knowledge of actions and usual dosages of most commonly used emergency drugs and their location on crash cart.Assess and document patient response to medication that have been administered.Instruct patient/family regarding prescribed medications/therapies and surgical/diagnostic procedures that shall be accomplished.Assess patient/family knowledge level, implement plan of instructions and record patient/family demonstrated level of understanding.Consistently apply VA customer service standards, demonstrating helpful, courteous, and respectful conduct at all times.QualificationsMust have graduated from a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the ACEN at the time the program was completed.A full, active, and current license in at least one (1) state, territory, or commonwealth in the U.S. or the District of Columbia.Certification in Basic Life Support (BLS).A minimum of two (2) years direct patient care experience within the last three (3) years.
