Generations Family Health Center, Inc. is a regional community health center covering a 37-town rural region in eastern Connecticut with sites located in Willimantic, Danielson, Norwich and Putnam. Our dedicated staff provides a full range of primary care services to all ages including medical, dental and behavioral healthcare, as well as care coordination and community outreach. Location : Putnam, CT (Behavioral Health Department & School Based Health Center) Responsibilities Under the direction and supervision of the BH Clinical Supervisor, conduct clinical evaluations and assessments, diagnose and provide treatment for patients. Conduct individual, group, family and/or couples therapy sessions for patients across the lifespan in accordance with accepted American Psychiatric Association guidelines for treatment techniques and therapies. Develop and coordinate a clinical treatment program for the diagnosis, treatment, or referral of patients presenting with behavioral health needs. Provide a multi-disciplinary approach to treatment plans, coordinating with medical providers and/or other behavioral health clinicians. Assess patient progress and modify treatment programs accordingly. Coordinate care of patients with referring physicians or agencies. Document all therapies according to established protocols. Attend required meetings and participate in committees as requested. Participate in professional development and maintain required continuing education hours for licensing. Maintain patient confidentiality and document case records on all contacts regarding patients. Qualifications Master’s degree in social work or related human service field, and experience working with individuals, couples and families. Licensed candidates preferred (LCSW/LMFT/LPC/LMSW/LMFTA/LPCA). Experience with Substance Use Treatment is preferred but not a requirement.
