Long Term Care Registered Nurse (CLC RN)

Job Summary Registered Nurse – Community Living Center (CLC) Position Overview Compensation: 50.00/hr plus $5.09 for Health and Wellness We are seeking a compassionate and skilled Registered Nurse (RN) to provide high-quality care in a Community Living Center (CLC) setting. This role focuses on short-term rehabilitation, skilled nursing care, and long-term support for Veterans transitioning from acute care to home or community environments. Primary Responsibilities Conduct comprehensive patient assessments and develop individualized care plans Administer medications, IV therapy, tube feedings, and wound care Provide tracheostomy care, catheter management, and chronic disease support Assist patients with activities of daily living (ADLs) Collaborate with interdisciplinary teams to support recovery and discharge planning Document care accurately in the electronic health record (EHR) Respond to medical emergencies and changes in patient condition Work Environment Post-acute rehab, dementia care, hospice, and skilled nursing units Patient population includes rehabilitation, respiratory, cognitive impairment, and end-of-life care Minimum Qualifications Graduate of an accredited nursing program (ACEN or CCNE) Active RN license (Ohio or compact state) Current BLS certification (ACLS required for ICU/ED) Minimum 2 years of recent clinical experience Strong English communication and documentation skills Additional Requirements Ability to work 12-hour shifts, including nights, weekends, and holidays Completion of VA-required training (safety, HIPAA, EHR, etc.) Compliance with credentialing and privileging standardsAbility to float between units as needed EEO Statement Aptive is an equal opportunity employer.
