Maintain patient and staff privacy and confidentiality pursuant to HIPAA. Assess assigned case load of home‑health care patients and families to identify physical, psychosocial, and environmental needs. Assume primary responsibility for caseload, including assessment, planning, coordination, implementation, and evaluation of patient care plans. Obtain data on physical, psychological, social, and spiritual factors that influence patient/family/caregiver health status and incorporate that data into care plans. Make initial and follow‑up evaluations of patient nursing needs during each visit, adjusting visit frequency as required. Communicate significant findings, problems, and changes in health condition or environment to the Clinical Supervisor, physicians, facilities, and other personnel involved in patient care. Revise the plan of care in response to identified issues and notify the Clinical Supervisor and team members. Report to the Clinical Manager or Clinical Supervisor. Requirements Current, unrestricted state RN licensee. At least 1 year of clinical nursing experience. Able and willing to travel within the branch/office coverage area; must be able and willing to travel 50% of the time. Ability to frequently lift, push, pull, and support up to 50 pounds. Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws. Associate’s Degree or Bachelor’s Degree in Nursing preferred. Previous Home Health or Hospice experience preferred. Previous experience with HomeCare HomeBase preferred.
