Pay Rate: $35.00 – $50.00 / hour (Based on experience)

Shift: 12-Hour Day Shift

Location: La Jolla, San Diego North County

What You'll Do (About the Role)

As a Private Duty LVN, you will deliver high-quality, one-on-one skilled nursing care to seniors and adults with disabilities in the comfort of their homes. We are looking for compassionate nurses with vibrant personalities who can blend exceptional clinical skills with genuine human connection.

Key Responsibilities

• Skilled Clinical Care: Provide exceptional hands-on care, including wound care, medication management, and the ongoing monitoring of chronic conditions.
• Care Coordination: Collaborate closely with physicians, rehabilitation services, and family members to ensure seamless, comprehensive care.
• Client & Family Education: Empower clients and their loved ones by educating them on diagnoses, treatment plans, and wellness strategies.
• Meticulous Documentation: Maintain highly organized, accurate, and timely clinical documentation of the exceptional care you provide.

Requirements

• Current, unrestricted California LVN license
• Graduation from accredited practical or vocational nursing program
• 1 year of experience as a Licensed Vocational Nurse
• Home Health experience preferred
• Proof of Negative TB result
• Physical Screening Documentation within the last year
• Current BLS certification
• Valid California drivers License and insurance
• Cell phone with internet access
