Antimicrobial Stewardship Clinical Pharmacist Specialist

Location: Phoenix, Arizona – University Medical Center Phoenix Schedule: Monday – Friday, 8am – 4pm Responsibilities Conduct prospective audit and feedback of restricted antimicrobial agents. Perform stewardship review of patients with bacteremia and ICU handshake stewardship. Serve as an infectious disease drug information resource for physicians, other providers, and pharmacists. Participate in education, quality improvement projects, and committee involvement at both the facility and system levels. Provide pharmacotherapy expertise to optimize drug selection, maximize therapeutic benefit, and minimize toxicity. Evaluate and document all components of pharmaceutical care delivered to patients and coordinate workflow of pharmacy technicians. Preceptor and mentor pharmacists, students, residents, and other health‑care providers. Apply continuous quality improvement principles to medication safety, drug use evaluations, and adverse event trending. Meet ASHP preceptor qualifications and serve as Program Director for the institution’s Pharmacy Practice Residency Program, if required. Minimum Qualifications Doctor of Pharmacy (PharmD) degree. Completion of a PGY1 residency program; *or* a minimum of 3years progressive clinical pharmacy experience in a health‑system setting. Active board certification in an appropriate specialty (e.g., BCPS, BCOP) – certified candidates have 24months to achieve certification if not yet certified. Valid pharmacist license in the state of practice; must be licensed within 90days of hire or complete the state board application within 30days of offer. Valid driver’s license (may be required for coverage under the company’s auto insurance policy). Proficiency with pharmacy software, Microsoft applications, and automated dispensing systems. Strong organizational, problem‑solving, and written and verbal communication skills. Good knowledge of medication management principles and the medication use process. Preferred Qualifications Completion of a PGY2 residency or PGY1 residency plus a minimum of 3years health‑system experience. Board certification in an appropriate area of specialty. Expert level working knowledge of principles, practices, and operations in the assigned area of responsibility. Additional related education and/or experience.
