Care Navigator

Position Purpose Develops, assesses, and coordinates care management activities based on member needs to provide quality, cost‑effective healthcare outcomes. Responsibilities Develop or contribute to personalized care/service plans for members, educating them and their families or caregivers on services and benefit options. Evaluate member needs, barriers to care, and available resources, recommending and facilitating plans for the best outcomes. Identify providers, specialists, and community resources needed for care and coordinate with them to ensure timely access. Provide psychosocial and resource support to members, caregivers, and care managers for accessing local services such as employment, education, housing, food, and justice. Monitor progress toward care plan goals, collaborating with healthcare providers to revise plans or address identified member needs. Collect, document, and maintain all member information and care management activities to ensure compliance with state, federal, and third‑party payer regulations. Perform on‑site visits to assess member needs and collaborate with providers or resources, when appropriate. Provide education to care managers and members on procedures, provider instructions, care options, referrals, and benefits. Perform other duties as assigned by the people leader to meet member and business needs, and comply with all policies and standards. Education & Experience Requires a Bachelor’s degree and 2–4 years of related experience. Preference for graduates from an accredited School of Nursing if holding clinical licensure, or equivalent experience that demonstrates applicable knowledge, duties, scope, and skill at this level. Licensing & Certification Current state’s clinical license preferred. Pay Range $22.94 – $38.79 per hour.
