Woodway Rehabilitation & Healthcare Center is seeking a compassionate  full-time Occupational Therapist (OT)to join our in-house rehabilitation team.

Consistent and Traditional PRN opportunities also available.

As part of our collaborative rehabilitation team, you will provide personalized, resident-centered therapy and develop individualized treatment plans that support meaningful progress in daily activities and overall function.

Key Responsibilities

• Evaluate residents’ functional abilities and their impact on daily activities
• Develop and implement individualized occupational therapy treatment plans
• Provide skilled therapy services focused on independence in activities of daily living and instrumental activities of daily living
• Recommend adaptive equipment and home modifications as appropriate
• Monitor resident progress and adjust OT treatment plans as needed
• Educate residents, families, caregivers, and clinical staff
• Collaborate with the interdisciplinary care team
• Maintain accurate, timely, and compliant documentation
• Ensure services meet all professional, legal, and regulatory standards

Qualifications

• Active Occupational Therapy state license in good standing
• Skilled nursing or long-term care experience preferred
• Strong clinical, communication, and organizational skills
• Knowledge of the Net Health Electronic Documentation System preferred
