The Avenue at Strongsville, a post-hospital rehabilitation and skilled nursing care facility, is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team. As a valued member of our healthcare community, you will have the opportunity to make a meaningful difference in the lives of our residents and patients.

About the Role:

As an LPN at The Avenue at Strongsville, you will be responsible for providing exceptional nursing care to our residents and patients. This includes:

• Assessing and documenting patient condition, including vital signs, medical history, and treatment plans
• Administering medications and therapies as prescribed by our medical team
• Developing and implementing individualized care plans to meet each patient's unique needs
• Collaborating with our interdisciplinary care team to ensure seamless coordination of care
• Educating patients and their families on their condition, treatment options, and disease management
• Maintaining accurate and timely records of patient care and treatment
