The care environment is more intensive than traditional skilled nursing or rehabilitation, allowing pharmacists to work closely with the care team to optimize advanced pharmacotherapy and support recovery over a longer timeframe.Your day-to-day responsibilities will focus on delivering direct clinical pharmacy services as part of a multidisciplinary care team. This includes reviewing medication orders for safety, efficacy, and appropriateness

verifying dosing based on renal and hepatic function

managing drug-drug interactions

and tailoring therapy to the unique needs of each medically complex patient. You will participate in patient care rounds, collaborate with physicians and advanced practice providers on treatment plans, and provide recommendations on antimicrobial therapy, anticoagulation, pain management, sedation, and chronic disease management.Pharmacists in this long-term acute setting play a critical role in therapeutic drug monitoring, particularly for medications with narrow therapeutic indices. You may oversee vancomycin and other high-risk medication levels, adjust dosing based on lab results and patient response, and ensure appropriate lab follow-up. Antimicrobial stewardship is a key aspect of the role, and you will help guide appropriate antibiotic selection, de-escalation, and duration of therapy in order to balance effectiveness with resistance prevention.Medication safety and quality improvement are integral to the position. You will be involved in identifying and preventing medication errors, participating in medication use evaluations, and contributing to institutional protocols and clinical pathways. Pharmacists frequently assist with medication reconciliation at admission, transitions between levels of care, and discharge planning to minimize readmission risk and ensure safe continuity of therapy.The patient population in a long-term acute care environment often includes adults who are ventilator-dependent, have tracheostomies, complex wound care needs, or long-term intravenous therapies. You will be exposed to a wide range of clinical scenarios, making this an excellent role for pharmacists seeking to further develop skills in intensive, medically complex care while still having the opportunity to build ongoing relationships with patients over their extended stays.Schedules may include a mix of day, evening, or weekend shifts, depending on facility needs, with a focus on maintaining consistent coverage to support safe, continuous medication management.
