This role functions as an integrated member of the Interdisciplinary Team (IDT), supporting participant health outcomes through assessment, care planning, coordination, education, and hands-on nursing services across clinic, home, and community settings.The RN ensures compliance with federal, state, and CMS PACE regulations while delivering participant-centered, high-quality nursing care throughout the participant lifecycle—from intake through ongoing enrollment.Education, Licenses, Certifications and Experience:Current Registered Nurse (RN) license in good standing (state-specific).Current CPR Certification.Valid Driver’s License (for field-based work).ACLS and UCAT certifications preferred (as applicable).Experience in geriatric, home health, or PACE preferred.Prior experience with in-home assessments and care coordination strongly preferred. #J-18808-Ljbffr VALIR PACE LLC
