Client Care Coordinator

Job Description: We are a growing home care agency seeking a professional, compassionate, and organized Care Coordinator to help manage and elevate our client experience. This role is ideal for someone who enjoys working with people, building relationships, and ensuring clients feel supported, heard, and well cared for. This is a hands-on role that involves both office coordination and in-field client interaction. About the Role: As a Care Coordinator, you will be responsible for: onboarding new clients maintaining ongoing client relationships ensuring quality of care and acting as a key point of contact for client needs You will play a major role in making sure nothing falls through the cracks. Key Responsibilities: Conduct in-home consultations (when nurse is unavailable) Gather client information and assess care needs Set up and maintain client files (digital + physical) Ensure all documentation is complete and audit-ready Client Follow-Up & Relationship Management Conduct ongoing client check-ins Build strong relationships with clients and families Address concerns professionally and proactively Compliance & Documentation Maintain organized, up-to-date client files Ensure all records meet compliance standards Field Visits & Quality Assurance Perform client visits as needed Identify and address any issues in the home Provide feedback to office team on client needs Team Collaboration Communicate client needs to scheduling and recruiting team Help ensure proper caregiver matching Support overall client experience and retention What We're Looking For: Strong communication and people skills Professional, calm, and solution-oriented Highly organized and detail-oriented Ability to handle difficult conversations with confidenceTeam player with a positive, proactive attitude Preferred Background: Healthcare, home care, or social work OR strong customer service / client-facing experience Sales or relationship-building experience is a plus (We are open to candidates who are quick learners and highly motivated) Requirements: Reliable transportation Valid driver's license Ability to travel locally for client visits Growth Opportunity: This role has the opportunity to grow into: a more field-based role leadership within client services or expanded responsibilities in client development Compensation: Competitive hourly rate (based on experience) Opportunity for growth and advancement Pay: $22.59 - $27.20 per hour Benefits: 401(k) Dental insurance Disability insurance Life insurance Opportunities for advancement Paid sick time Paid time off Paid training Vision insurance Work Location: In person #J-18808-Ljbffr Compassionate Connections Home Care
