Hospice Field Case Manager / RN / Registered Nurse
Compensation and Benefits

Salary:

• $115,000 - $125,000
• $15,000 Sign on Bonus
• Excellent Benefits!!!

Location:

• Territories available: Bronx, NY or Manhattan, NY

Position Summary
When days cannot be added to a life, we add life to remaining days. Our multidisciplinary teams offer culturally-sensitive, end-of-life and palliative care services to patients and their families at home or in assisted living communities, skilled nursing facilities and hospitals.


Training Provided for RNs with strong clinical bedside experience!

Responsibilities

Key Duties:

• Visit hospice and palliative care patients in community and facility settings to perform physical and psychosocial assessments, and document visits electronically.
• Develop, implement, and update individualized nursing care plans based on patient needs.
• Coordinate care changes with physicians by requesting written orders when needed.
• Supervise, train, and evaluate home health aides and homemakers supporting hospice patients.
• Follow the required schedule, including one weekend per month and backup on-call coverage 2-3 times monthly, with car and escort service provided for on-call shifts.

Qualifications

Education:

• Graduate from an accredited School of Nursing, (BSN preferred)

Licenses and Certification:

• New York State RN licensure
• Valid NYS Driver's license required

Skills:

• Two years of Community or Oncology nursing experience
• Working knowledge of Word and Excel
• Bilingual a plus!
