Ultimate Therapy is hiring a Physical Therapist (PT) to provide rehabilitation services at Autumn Lake at Norwalk. Join a supportive clinical team focused on helping residents restore movement, increase strength, and make meaningful progress toward their functional goals.
What You'll Do

• Perform comprehensive physical therapy evaluations and establish individualized plans of care
• Provide skilled treatment addressing mobility, strength, balance, gait, transfers, and endurance
• Establish functional goals and track resident progress throughout the rehabilitation process
• Modify treatment plans based on clinical findings and response to therapy
• Educate residents and caregivers on exercises, mobility techniques, and fall-prevention strategies
• Complete required clinical documentation accurately and on time
• Coordinate care with PTAs, OTs, SLPs, nursing staff, and other healthcare professionals

Qualifications

• Active Connecticut Physical Therapist license required
• Graduate of an accredited Physical Therapy program
• Strong clinical judgment and interpersonal skills
• Post-acute or skilled nursing experience is a plus, but not required
• New graduate Physical Therapists are welcome to apply
