What You’ll Do Complete Start of Care (SOC) admissions efficiently and accurately , balancing quality with productivity. Perform clinical assessments and OASIS documentation with speed and precision. Initiate care plans and quickly identify needed services and next steps . Communicate effectively with the care team to keep patient transitions moving smoothly . Manage your schedule and territory to optimize time, travel, and visit volume . What You Bring Active Minnesota RN license in good standing. Experience with home health admissions and OASIS strongly preferred. Ability to work independently in a fast-paced, field-based role . Strong time management and organizational skills . Reliable transportation. Additional Details Mileage reimbursement provided. Territory-based scheduling across the Twin Cities Metro.
