Job Details

• Position Description: Outpatient PT providing therapy services based on physician direction and patient needs. Responsibilities include conducting comprehensive evaluations, developing individualized treatment plans, and ensuring timely documentation of patient care.
• Required Qualifications: Degree in Physical Therapy from an accredited school, current CPR certification, active applicable license for the state where work will be performed, 6 months to 1+ years of outpatient clinical experience, and a minimum of 1 year of experience.
• Schedule: Days shift, 8-hour shifts, minimum of 40 hours per week.
• Additional Information: Performs evaluations and develops treatment plans to progress patients towards discharge within established length of stay. Completes all documentation accurately and timely, ensuring compliance with state, federal, and intermediary guidelines. Participates in student programs, and actively pursues ongoing professional development. Adheres to all regulatory and safety standards, maintaining licensure in good standing and following the organizations performance improvement initiatives. Demonstrates commitment to patient and employee safety, including safe patient handling, infectious disease protocols, and slip/trip/fall prevention.
